How do we add this to our website?
Put a “Refer a patient” button or link on your site that opens your Dentadex referral page. It's one link, so your web person adds it like any other link. Dentadex can email them the button and the steps.
How do referring offices find our form?
Three ways: the button on your website, the link in your emails and email signature, and the same link on anything you hand out. The link never changes.
Why is it a Dentadex page and not a form on our own website?
Most website builders don't sign a BAA for their forms, and form plugins email what people type in plain text. Patient details and X‑rays can't go through that. Your Dentadex page is HIPAA-compliant: encrypted, every file scanned, covered by our BAA, and it carries your name and logo.
Does the referring office need an account?
No. They open your link, fill in the form and send it. No account, no password, nothing to install. If they leave an email, they get a receipt with a reference number.
Can we change what the form asks for?
Yes, yourself, in your settings. Every field can be required, optional or hidden, and you can require X‑rays so a referral can't be sent without one.
Who gets told when a referral comes in?
Anyone you choose. Add as many email addresses as you like and each one gets the referral in the email. Text alerts carry only a link.
What can they attach?
X‑rays, photos, ID and insurance cards, PDF letters. JPG, PNG, HEIC or PDF, up to 20 MB each. Every file is scanned before anyone on your team opens it.
